Medical and Media Release

Note to all parents:

As our ministry has grown (praise the LORD!), we have noticed the redundancy of asking for medical information more than once a year. This has not only impacted you, but it has also impacted our office staff.

In an effort to make filling out and tracking forms as EASY as possible we have decided to implement an ANNUAL Medical and Media Release. This form will be valid from September 1 to August 31 each year.

We are asking that EACH student has a Medical and Media Release on file with Amy Pavlo prior to Wednesday September 23

This is important for you because:

• A form MUST be on file before your student can attend ANY Friendship Church event on or off campus. (Zone Corn Maze, Fall Retreat, Turkey Bowl, Winter Retreats, Winter Bible Institute, FROG Camp, etc.)
• Having the form on file will also help us with our regularly scheduled programs on Wednesday nights and Sunday mornings in the case that emergency care for your student is necessary.
• The form MUST be updated if your medical insurance, doctor or dentist change or if there are any major changes in your student's medical condition.

This is important for us because:

• It updates your student's information in our system annually so we can be sure to communicate well.
• Knowing a student's medical conditions helps us to best serve your child.
• In case of emergency it allows us to legally treat your child and to get him/her to a medical facility as quickly as possible when/if necessary.
• It allows permission for your student to be treated at the medical facility.
• It informs medical personnel of any medications your student is taking in order to avoid harmful medication combinations and to give your student the most appropriate and speedy care.
• It also allows us to post photos and video of your student on our website, brochures and other advertising vehicles.
